Would you like to contribute to improving quality within the services offered by Bradford District Care Foundation Trust? If so then read on…..

Every year we set ourselves a number of Quality Goals, achieving these goals leads to improved quality, safety and experience for people who use our services.

We have designed a survey to gather your views which will help us set our goals for 2016 / 17; we would like to know what quality issues you feel are important and / or what  we could do better. 

We will publish the Quality Goals in our annual Quality Report, monitor them throughout the year and report on our progress in the next annual Quality Report.

What is a Quality Report? All Foundation Trusts are required to publish an annual Quality Report (or a Quality Account for non-Foundation Trusts). This is a report about the quality of services.  It includes information about how well we are doing against the Quality Goals we set the year before.  It also includes information about the things we intend to make a priority for the following year.

We would really appreciate your contribution so please take a few minutes to complete this survey. The survey responses will be analysed to support us in agreeing our Quality Goals which will be published on our web site as part of the Quality Report.  

Your response is anonymous and feedback will be combined to help us to identify an appropriate set of Quality Goals for 2016/17.
