Call for evidence: Reducing health inequalities for people living with frailty

As part of the VCSE Health and Wellbeing Alliance, we are delivering a project which aims to help the NHS to identify: - Why there may be inconsistences in delivery of healthcare services to disadvantaged people experiencing frailty earlier in life. - Opportunities to reduce and overcome the barriers to accessible health care that disadvantaged people experiencing frailty earlier in life face. - How services and examples of good practice have successfully overcome the barriers to health care services that exist.
- Opportunities for health and care services to support people and to prevent the early onset of frailty.
As part of this project we are issuing a call for evidence for examples of best practice for services that have improved access to and experience of health care services for disadvantaged people living with frailty much earlier in life. This includes within health and social care, the voluntary sector, grassroots community groups and more.
How to respond
Please share any examples of work being undertaken currently or previously. These examples will be used to identify and highlight effective practice. If you are engaged in multiple projects, please complete a separate questionnaire for each activity. If the information for any of the sections is not available, please leave them blank.
