* 3. I declare that I am qualified to be enrolled according to the provisions of the Cathedrals Measure 2021. Please tick the below options that apply:

A) I am 16 or over and baptised and a member of the Church of England or a Church in communion with it, and an habitual attendee at public worship at the Cathedral in the last six months

B) I am 16 years or over and baptised and a member in good standing of a Church which is not in communion with the Church of England but subscribes to the doctrine of the Holy Trinity, I am also a member of the Church of England, and an habitual attendee at public worship at the Cathedral in the last six months

I am not eligible to register under points A or B above, but I am a member of the Cathedral community (e.g. staff, volunteer)

I do not wish to be part of the Community Roll from 2024 to 2029
