This survey can be completed by anyone who has experienced serious injuries in a road crash, or by anyone who has cared for someone injured in this way.

We hope you can tell us a little bit about what happened to you during and after your crash, about the medical care you received at the time as well as longer-term support you have needed. We are particularly interested in finding out about the information and advice you received after the crash, and how it helped you to cope.

All questions can be answered anonymously and you can choose which questions to answer and how much additional information you want to give.
