Introduction
This survey collects evaluation data about Reading Well activity in your library authority taking from April 2017 to March 2018. Because we have a good amount of data relating to previous schemes the focus of this year’s survey is on the long term conditions scheme, although there are some general questions about all strands of books on prescription.
The evidence will be vital to help The Reading Agency and Society of Chief Librarians improve the overall programme and to build the case for future investment - we will use the results for programme development, fundraising and advocacy. The findings will be considered alongside data from prescribers, service users and partners as part of an annual evaluation report. The report will be made publicly available on our website this autumn.
We would be grateful if you could complete this survey by Friday 27 July.
