As part of the NHS Five Year Forward View, local health and social care organisations have been invited to work together to develop a vision and priorities to address local challenges and to improve the health and wellbeing of people in our community.  These plans were initially in 2016 called Sustainability and Transformation Plans but have now become Sustainability and Transformation Partnerships (STPs).

North East Lincolnshire is part of the `footprint’ area of Humber, Coast and Vale which stretches from Scarborough in North Yorkshire, through East Riding and Hull  to North Lincolnshire and North East Lincolnshire.

Healthwatch North East Lincolnshire would like you to complete this questionnaire because it will help to make sure that the views of local people are taken into account in the planning and development of future health and social care provision locally.
