Welcome to The FDF's (formerly known as Flintshire Disability Forum) Survey

This survey is being conducted by The FDF (formerly known as Flintshire Disability Forum) to find out the issues which you are facing during the pandemic and how to help moving forward. Please read the information below carefully and ask questions to clarify any information. The survey should take 5 minutes to complete.

Do I have to take part?
No, your participation is completely voluntary. None of the questions are compulsory, only answer questions you are comfortable answering. You can stop the survey at any point and withdraw your data until it is submitted.

Confidentiality will be adhered to under the Data Protection Act 2018. There will be no way to track your answers to you, unless you provide your contact details. All details will remain strictly confidential. Answers cannot be removed once they are submitted. 

What will the data be use for?
Your answers to will be written into a report that will be used to bid for funding to ensure what your needs are met.

Risk of taking part:
There is no harm which should occur from completion of the survey.

Benefits of taking part:
Your voice on how to move forward during the pandemic will be heard.
