Thank you for giving us a few minutes of your time. Sharing your experiences with us will take you no more than a few minutes to complete. It is imperative for us to collate feedback from our families so that we can ensure our families lived experiences and voices inform and influence the future landscape and opportunities for people with a learning disability and autism and their families in Essex.
 
We encourage you to come back again if and when you have further experiences to share. Because we do not store identifying data, each time you come back you may feel you are ticking boxes you have already ticked, this ensures we are keeping our insight up to date and would ask you to spare those few moments to do so.

By providing consent, you are agreeing that personal information about you will be collected anonymously and stored securely and will not identify anyone personally. The anonymised data will be used and shared with relevant
organisations for the purpose of improving opportunities for people with a learning disability and autism and their families within Essex and England but will not identify anyone personally. 

Once consent has been provided you have the right to withdraw your views and your views will be removed, by emailing info@essexcarersnetwork.co.uk, however where data has been anonymised and amalgamated in certain
circumstances it may not be able to be excluded.
