1. Welcome

Thank you for participating in our survey.

Dear Participant,

This is an invitation for you to complete an online survey conducted by researchers from the University of South Australia, in partnership with Ovarian Cancer Australia.  This project is focused on The Impact of Ovarian Cancer on Quality of Life. In particular, the purpose of this research is to establish the prevalence of sleep disturbance amongst women who have been diagnosed with ovarian cancer, compared to women without ovarian cancer. This study also aims to investigate the impact of sleep disturbance and work-related activities on quality of life and to quantify the financial burden associated with ovarian cancer.

Participation in this survey is voluntary. Women over the age of 18 who have been diagnosed with ovarian cancer are eligible to participate in this study. Women without a diagnosis of ovarian cancer or another chronic illness (other cancers, chronic pain conditions, diabetes, cardiovascular disease, sleep disorders, dementia, or arthritis) are also eligible to participate and will be part of our comparison group for this research project.

You will be asked a series of questions which will take approximately 20-25 minutes to complete. If you have any queries after reading the information provided, you may contact either of the researchers via email using the details below before participating in the study.

The survey will contain questions asking for personal information pertaining to your cancer treatment and impact (if applicable), and lifestyle (including sleep and general wellbeing). To help us understand the financial impact of ovarian cancer on Australian women, we will also ask you some questions about your work history and household income.

It is not anticipated that there are any risks to participating in this study beyond those encountered in everyday life. However, some questions may be confronting or sensitive in nature. If participation does trigger any adverse responses, you are advised to call Ovarian Cancer Australia for cancer specific information on 1300 660 334 or Lifeline for psychological support on 13 11 14.

Your participation in this study will be anonymous including all records containing personal information or specific demographics. You may withdraw from the study at any time without consequences.

Although the researcher will take every care to keep data confidential, survey results will be stored on the Survey Monkey platform and the manager of that platform may have access to the data. However, since data collected is anonymous, the platform manager will not be able to identify participants. As soon as practical after data collection is complete, the researcher will delete the data from the online platform. However, confidentiality of anonymous data stored on the Survey Monkey platform cannot be guaranteed. Once the survey has been submitted, we will be unable to remove your response as it will be impossible to identify your individual data. Please note that non-identifiable data collected in this project may be used in future research projects, for which ethics approval will be sought.

Information collected from the study will be kept for a period of five years post study completion. Unless required by law, all records will be stored confidentially, and the data will not be individually identifiable.
