You are being invited to take part in a research project based on your perceptions on the use of end of treatment certificates to support their introduction in UK Radiotherapy departments.

Before you decide to do so, it is important you understand why the research is being undertaken and what it will involve. Please take time to read the following information carefully. If there is anything that is not clear or if you would like further details please do not hesitate to ask.

The use of end of treatment certificates signed by staff members is carried out in the Princess Margaret Hospital, Toronto, Canada. This has shown to be a positive way to mark an important milestone in their patients’ lives.

The purpose of this research study is to determine whether end of Radiotherapy treatment certificates could be beneficial in UK Radiotherapy departments.

The results from the research will help fill a gap in research on methods of facilitating patients’ sense of achievement and hence motivation on their final day of treatment.

The study is aimed at previous radical patients 18 and over, who have completed a course of Radiotherapy, and will therefore have the relevant knowledge and insight about the questions asked.

The questionnaire is completely voluntary and should you wish to withdraw from participating you are able to do so at any point.

The study will involve completing a web-based questionnaire which we estimate will take you under 10 minutes. The questionnaire will ask you about your opinions in relation to an end of Radiotherapy treatment certificate.  Your views and experience based on this certificate are just what the project is interested in exploring.

Furthermore, all the information collected will be completely anonymous and confidential. Please note that in submitting the completed questionnaire, you are consenting to having your answers included in the study.

This project has been ethically approved and reviewed by The School of Healthcare Sciences, Cardiff University, Research Ethics Committee.
