This survey is being undertaken by the NW London NHS maternity service.
Our maternity teams want to ensure that all users of midwifery services in NW London have the right access to education, support and information with regard to pelvic health, during and after pregnancy.
We appreciate that this can be a very sensitive and private topic of conversation, and we would like to reassure you that all the information submitted in this survey is anonymous and will be used in the strictest of confidence.
Any information you feel comfortable in providing will be used for the purposes of improving the level of service and support we can offer all women with pelvic health complications.
We would like to hear the views of anyone who has given birth in the last 12 months and those who are currently pregnant.
