Pupil Premium Parent Questionnaire

Dear Parent,

Before the summer your child was given a Pupil Premium Pack that contained:

School uniform – 2 blue Gayton sweatshirts/cardigans,

2 white polo shirts and a skirt/pair of trousers;

PE kit - a white t-shirt and blue shorts;

all trips paid for - including residentials;

a free book at each book fair – to encourage reading;

subscription to First News – a children’s newspaper;

a Gayton book bag and water bottle;

a PE bag;

a pencil case;

stationery and paper – to encourage creativity and to help with homework;

a year’s supply of Omega 3 supplements – proven to improve mental ability and memory;

a Kindle – to encourage reading;

an alarm clock- to help with the learning of time.

I am very interested to know what you thought of the pack. I would really appreciate it if you could spend a few minutes completing this form and returning it to me, as this will help to inform us as to what to put in future packs.

Many thanks in anticipation,

Mr O’Donnell
