This form must be submitted by 25th March 2016.

Judging criteria:

Entrants must write a 500 word case study or a summary of their initiatives to provide excellent customer service to consumers. The case study or summary must include:

• Speed of answering customer queries
• Channels of communication
• Contract options and flexibility
• Handling of complaints
• Speed of dispute resolution
• Anti-abuse services
• Corporate social responsibility credentials
• Safety and security features and accessibility
• How your customer service differs from that of competitors
• Innovation in billing and customer contact
• Examples of ‘going the extra mile’
