Our Trustees will be working this year to decide where our budget should be prioritised and what services we will therefore be able to offer the SMA community from April 2018 - March 2023. To help them do this, we would like to know what health, education and social care professionals think of our current services and how they have used them. 

The survey should take about 10 minutes of your time.

Any identifying information will be removed and answers will be collated with other replies. All the collated information will be discussed with our staff and trustees and will help shape the future of our services. It may also be shared with our funders and form part of our public Annual Activity Report. 

The closing date is 5.00 pm Wednesday  May 31st 2017.
