Registration with Nikii Fenton

* 5. By registering for classes online, I agree and consent to the following: I am voluntarily participating in fitness classes with Nikii Fenton. I am aware of hazards and risks involved. I understand it is my responsibility to consult with a physician prior to and regarding my participation in any this activity. I agree to assume full responsibility for any risks, injuries or damage known or unknown which I might incur as a result of participating in the program. Such injuries can include but are not limited to, heart attacks, broken bones, shin splints, muscle strains, tears or pulls, injuries to knee, back, shoulder, foot or any other illness or soreness including death. I knowingly, voluntarily and expressly waive any claim I may have against Nikii Fenton and Zumba Fitness LLC for injury or damages that I may sustain as a result of participating in this program. I /my heirs or representatives forever release waive, discharge and covenant not to sue Nikii Fenton and/or Zumba Fitness for any death or injury caused. My agreement also acknowledges that I may be videotaped, audio recorded or photographed during the Activity or event, and Nikii Fenton may use and modify these images and/or recordings for all uses including, but not limited to advertisements and marketing without any compensations and in perpetuity. I voluntarily agree to the terms and conditions stated above and understand that by taking part the class is the equivalent of signing my name.
