NHS Vale of York Clinical Commissioning Group (CCG) plans and pays for healthcare services for people living in the York area.

We are working with partners to create an end of life care strategy that strengthens our commitment to improve and develop end of life care and support services.

We would like to capture the experiences of anyone who cares for someone at the end of their life in a professional capacity.

We are asking clinicians and professionals to complete a questionnaire using the six ambitions (taken from Ambitions for Palliative and End of Life Care: a national framework for local action 2015-2020).  From these six ambitions we have created ‘I’ statements to help us capture what we do well as a health and social care system, and what is still needed to realise these ambitions.  

We will use your responses to inform the content of the strategy and create a Citizens’ Charter that reflects the needs of people across the Vale of York.
