Dementia Friendly Haywards Heath Survey

Our vision is to create a dementia friendly community where people living with dementia and their carers ​are active participants within our community, where they can remain independent within a safe and supportive environment. 

Over the next few months we would like your help in learning more about what support and care is needed to 'Live Well with Dementia' in our community. Your honest answers will help us structure and plan how Haywards Heath DFC can best support you and your loved ones, and make our community more dementia friendly.
