You have recently received support from the Hackney Children and Families Service Children in Need Team.
We are sending out this survey to better understand what you think we do well and, what you think we need to improve on.
We would appreciate you taking the time answer the questions. It should take about five minutes.
We will not tell anyone about your answers unless you say something that concerns the welfare of a child.
