Community Diabetes Service – Patient Feedback Survey

NHS Bromley Clinical Commissioning Group is seeking your views on how we can improve diabetes care for our population. Currently, Bromley has a very specialist community diabetes service which has consultant clinics, nurse specialists, and GPs working closely to deliver high quality care.

The current contract for this community diabetes service is due to expire and we are reviewing what needs to be provided before we re-procure the service.

This survey is for people with diabetes or carers of people with diabetes, who have accessed Bromley diabetes services.
