Community Champion (CC) Project Survey

What is the CC project?

The CC project is run by charity Communities 1st and is funded by Welwyn Hatfield Borough Council. Its aim is to increase the COVID-19 vaccination uptake in Welwyn and Hatfield by recruiting volunteers to become Community Champions.

These Champions are asked to speak with their local communities about COVID-19 vaccinations and to feedback to the charity any relating opinions, questions, or health information requests.

If communities need reliable health information in accessible formats, the charity will pass this to the Champions to disseminate. If communities need transport to vaccination clinics, the charity will put them in touch with local community car services, or if there is a big enough group, we may also be able to bring a vaccination clinic to you!

If you are interested in signing up as a Champion, there will be an option to add your contact details at the end of the survey.

This survey is for information gathering purposes, and all answers will be anonymised, so please answer as honestly as you feel able.
