The questionnaire is part of a research project evaluating the Green and Healthy Futures project. The research is being led by Veronica Wignall from Frome Town Council in partnership with researchers at the University of Plymouth. This research has been approved by the University of Plymouth Faculty of Science and Engineering Human Ethics Committee.

Your responses will remain anonymous and all information collected during the survey will be stored securely and treated confidentially. Your participation is voluntary, and you can choose not to complete the survey. The data we collect may be used for research and evaluation purposes, and might be used and presented in academic research and/or promotional or shared learning materials. In these cases, all data will be anonymised and no identifying or personal information will be used or published. Since your responses will be anonymous, it will be impossible to withdraw your data once you have submitted the survey. You must be over 18 years of age to take part in the survey.
