Consent Form

Thank you for following the link to this survey for the EHealth Productivity and Innovation in Cornwall and the Isles of Scilly (EPIC) Project.

The purpose of this survey is to understand the barriers and enablers to technological diversification. For this survey, technology diversification is a business strategy to increase market penetration through the development of new devices that combine software and hardware (e.g., wearable technology, sensors, robots, electronic devices).

This is a research project being conducted by the EPIC project at Plymouth University in collaboration with Software Cornwall. You are invited to participate in this study because you have been identified as a business with a significant element of software development involved in what you do.

Depending on your answers, the survey will take around 10 to 15 minutes to complete. The survey has three sections (i) your business, (ii) technology diversification, and (iii) eHealth.

Information will be stored securely within the SurveyMonkey system in a password controlled account belonging to the EPIC project and on password controlled Plymouth University computers and subject to the Data Protection Act and all other relevant legislation.

Although your views will help us in conducting this important research, you do not have to take part if you do not wish to. If you decide not to participate in this study or if you withdraw from participating at any time, there will be no impact on your relationship with EPIC or University of Plymouth.

The survey will close on 01/06/18, and initial analysis of the data will commence on 04/06/18. However, if you decide that you do not want us to include your responses in the research, even after completing, or partially completing the survey, please send an email to Gabriel Aguiar at gabriel.aguiar@plymouth.ac.uk until 30/06/18, who will then remove your data.

By completing the survey, you are giving us consent to use the information.
