What this survey is about

We would like to invite you to complete this short online survey which aims to describe DAPT prescribing practice in the UK and identify factors that cardiologists prioritise when prescribing antiplatelet agents for their patients.
 
You are invited to participate in this survey because you are a cardiologist who prescribes DAPT to patients post-PCI or ACS. Your participation is voluntary. If you decide to participate, you may withdraw at any time. The survey will take about 20 minutes to complete.
 
Your responses will be confidential. We will not collect identifying information such as your name, email address or IP address. However, all participants are eligible to enter a prize draw to win a 10.5-inch iPad Pro 64GB. If you wish to enter the prize draw please leave your email address at the end of the survey. The winner will be notified by email. All data will be stored in a password protected electronic format. Once the prize draw is complete, we will permanently delete the email address you provided.
 
The results of this study will be used for research purposes and will not be shared with anybody outside the research team. This survey is part of a research study entitled Clinician and patient factors influencing dual antiplatelet therapy (DAPT) prescribing in patients undergoing coronary interventions in the UK: a mixed method study, which is funded by the National Institute for Health Research (NIHR) Health Technology Assessment (HTA 14/192/89). This research has been reviewed and approved by the Health Research Authority (HRA) and the South West Cornwall & Plymouth Research Ethics Committee (17/SW/0092).

We will be using information about your clinical practice in order to undertake this study and will act as the data controller for this study. This means that we are responsible for looking after your information and using it properly. University Hospitals Bristol NHS Foundation Trust will keep your email address (the only identifiable information we will collect from you if you provide it because you want to be entered in the prize draw) until we complete the prize draw (one week after the survey closes). Your rights to access, change or move your information are limited, as we need to manage your information in specific ways in order for the research to be reliable and accurate. If you withdraw from the study, we will keep the information about you that we have already obtained.
