2016 is the Year of the Dad and PAMIS recognises the importance of fathers who contribute so much to family life and are very much involved in caring for their son or daughter who has profound and multiple learning disabilities.  Fathers are often, due to work commitments etc., unable to attend meetings/events/workshops to gain a better understanding of what Self-Directed Support is and how important their role is in helping their son or daughter access the best services available.

With this in mind, the PAMIS SDS project team is seeking to engage more with fathers over the next 12 months.  We would like fathers to provide us with feedback to give us a better understanding of how you would want to engage with PAMIS and what that engagement might look like.

We would be like you to answer this short questionnaire.  As we appreciate that questionnaires are not most people’s idea of fun, we’re offering a £20 Amazon gift card to a lucky winner whose name will be drawn from a hat.  Please complete no later than 31st August 2016 to be in with a chance of winning and remember to include your phone number so that we can notify you if you are the prize winner!
