Now that you have fully read / started to use / are fully familiar with the Hypo pack, we ask that you complete again the following short survey of 10 questions
 
Context: 
In order to evaluate the pharmacy teams improving knowledge of hypoglycaemia and how to support at risk patients in practice we want to gauge the new Diabetes Hypoglycaemia Support Packs' relevance and usefulness to you in your community pharmacy role.
 
Your pharmacy had a visit during the summer from one of the West Hampshire CCG's Medicines Management team, during which they talked you through the pack and left you a copy. The pack was developed and supported by Pharmacy Complete, MSD, West Hampshire CCG and CPSC. We hope you are finding it a useful development / reference tool to use in discussions with people with diabetes.

Rationale:
We are repeating the survey again for the people that attended the Diabetes Hypoglycaemia Development Events during October, to enable us to measure the change in responses as a result of using the Hypo Pack and attending the events. This will provide valuable insight into producing packs of this nature for use in community pharmacy.
