Introduction

The Royal Borough of Greenwich currently commissions a number of services from GPs in the borough:

· NHS Health Checks (this is a health check-up for adults aged 40-74. It's designed to spot early signs of stroke, kidney disease, heart disease and diabetes)

· Stop Smoking Support (services staffed by expert advisers to provide a range of proven methods to help people quit, including nicotine replacement therapy)

· Some specialist contraception options (such as the Implant or IUD, commonly known as the coil), but excluding the contraceptive pill.

GPs are not required to provide these specialist services. Some GPs choose to provide all of these services to their patients, whilst others provide one or two of them (for example, some provide Stop Smoking Services, but do not offer specialist contraception to their patients). There are some GPs that offer none of these services. This means that depending on which GP you go to, you may not have access to some of these services at your general practice.

We are planning to offer these services in a new way. We are proposing to establish up to 8 Live Well Greenwich hubs, based in general practices across the borough. These hubs will operate as separate clinics, open to all irrespective of which GP you go to.

We believe these hubs will provide convenient, speedier access to these services and we would like your feedback to help shape how they work.

The questionnaire will take between 5-10 mins to complete. Thank you.

* 1. Which of the following applies to you?

* 2. If a current or former service user, which of the services have supported you? (tick as many as apply)

* 3. Our proposal is to create up to 8 Live Well Greenwich hubs in GP surgeries across the borough, where people can go to get Stop Smoking Support, have a NHS Health Check or get Specialist Contraception. The hubs will be open to all (you won't need to be registered at the surgery to access the services). To what extent do you support or oppose our proposal?

* 4. Thinking about the design and delivery of the proposed Live Well Greenwich hubs, to what extent do you agree or disagree with the following?

Strongly agree Agree Neither agree nor disagree Disagree Strongly disagree N/A or don't know
The hubs should operate with set appointment times (i.e. you need to book an appointment in advance)
The hubs should operate as 'walk-in' (i.e. no need to book an appointment)
I would prefer that the Specialist Contraception clinics (Coil, Implant fitting) are delivered by female staff
Services should be available out of hours / weekend
I would go to a GP surgery to access these services

* 5. There may be opportunities to deliver other services from Live Well Greenwich hubs in the future. To what extent do you support or oppose the delivery of these additional services in hubs?

Strongly support Support Neither support nor oppose Oppose Strongly oppose Don't know / not sure
Sexual Health / Sexually Transmitted Infection (STI) Testing clinics
Immunisation and Vaccination clinics (children's clinics, holiday clinics, etc)
Weight Management Services (support to maintain a healthy weight)

* 6. Are there other services you would like to see delivered from the Live Well Greenwich hubs, based in general practices?

* 7. Which of the following transport methods do you regularly use?

* 8. How long would you be willing to travel to access one of the Live Well Greenwich hubs?

* 9. Finally, we would like to ask you a few questions about you. This information will help us develop and plan our services.

What is your gender?

* 10. What is your age?

* 11. Do you consider yourself to have a disability?

* 12. What do you consider to be your ethnicity?

* 13. How would you describe your sexuality?

* 14. Thank you for completing this questionnaire. If you further comments about our proposal, please provide them here.
