Maternity Continuity of Carer September 2018

Please fill in this questionnaire if you've had a baby in the North East of England in the last year or so. It will only take a couple of minutes and is designed to find out if mums are currently receiving 'continuity of carer' when they have a baby. If you have other children, please only consider your youngest child when you are answering the questions.

Yes, the same person was with me all the way through

No, there was a shift change so the midwife at the delivery was not the one I was introduced to after admission

Yes - I knew the person who did my first postnatal home visit well

Yes - I always knew who was a doctor, midwife, anaesthetist, nurse, maternity care assistant or other specialist role

Yes - it was mostly clear what each health care professional's role was

I knew who was the midwife but the other people involved in my care weren't always clear about their role

I sometimes knew but it wasn't always clear

I thought I knew at the time but later realised that some people were not who I thought they were
