Mental Health Services for 18-25 year olds

The information you provide in this form will be treated in the strictest confidentiality and will not be shared with any outside organisation without your direct consent.

Kent Clinical Commissioning Groups (CCGs) would like to improve the quality of the support offered to young people when they make their move to adult mental health services.

Currently mental health services for children and young people end at 18 years: this is the time when young people have to start using adult services.
We would like to know your view and experiences to help us improve community mental health services for young people aged 18-25. We are also looking for people to help us in shaping these services in the future.

Please complete the following short questionnaire and have your say on what needs to change and what needs to remain to help young people move into adult mental health services.
