All health and care organisations across the city and county have worked together to develop a plan called Joined Up Care Derbyshire (our Sustainability and Transformation Plan, or ‘STP’) which sets out our vision and a set of priorities to address a number of challenges and improve the health and wellbeing of people in Derbyshire.

Reasons why we think we have to change, include:
  • Care can sometimes be disjointed from one service to another because our hospitals, care homes, general practices, community services and voluntary sector don’t always work as closely as they should.
  • Some people are admitted to hospital beds who could be cared for in the community if the right support was in place. There are growing waiting times for many services and access to primary care needs to be better.
  • There are new drugs and ways of treating people - including ever advancing technology.
  • People are living longer than ever before, which means we are treating more people into their later years. This means there is higher demand for NHS services.
  • There won’t be enough money - if we don't change the way we work now, we will have a gap in funding of approximately £219 million  in Derbyshire’s health system and £136 million in locality authority care costs by 2021.
  • Some illnesses we treat can be prevented by having support to live healthily (for example, by not smoking or drinking too much alcohol).
 
For all the reasons above, the people involved in Joined Up Care Derbyshire think that the time is right to work together to develop new ways of providing services across the city and county. 
 
We would like to invite people to answer the questions below so we can find out what people think about the reasons we need to change and what their ideas are around the priorities we have set out. This questionnaire is a small part of the work we are doing to talk to people in Derbyshire about how we can start to consider how we shape the future of health and social care.

The questionnaire should only take 10 minutes of your time and we really appreciate you taking the time to fill it out.
