Your local Clinical Commissioning Groups (CCGs) are responsible for buying health services for patients in your area. In North Yorkshire there are four CCGs:
  • NHS Vale of York CCG

  • NHS Harrogate and Rural District CCG

  • NHS Scarborough and Ryedale CCG

  • NHS Hambleton, Richmondshire and Whitby CCG

The CCGs and North Yorkshire County Council are responsible for ensuring that local health and social care equipment services meet the needs of those who need them. This includes some people also living in the Airedale and Cumbria areas of North Yorkshire, due to council overlaps.

We are looking at how these services can be improved in the future. To do this, it is important that we gather the views of people who currently use services.

Please complete this survey to help us understand what is important to you when accessing community equipment and wheelchair services.

Your contribution will help inform what these services will look like in the future.

This survey closes on Friday 29 January 2016.

The results of this survey will be available in March 2016 on the North Yorkshire County Council and local CCG websites. Paper copies of the results will also be available on request.
