Chief executive team award
A team who consistently demonstrates good team working to deliver an efficient and high performing service. They will have successfully improved services and/or implemented change directly, or ultimately, for the benefit of patients.

Compassion award
An individual or team showing a genuine concern and compassion for others – whether that be a patient, relative, colleagues - or all three.  They are interested in others, treat people as valued individuals – showing care and compassion.  They strive to put the needs of others before their own as part of providing the best possible care or service delivery.  

Excellence in care – clinical award
An individual or team that goes beyond their job description to deliver the best possible patient care.  They will be dedicated, determined, demonstrate excellent customer care, consistently exceed expectations, think creatively and solve problems for the benefit of patient care and safety.  They will also be dedicated to quality improvements and efficiency within their service.

Excellence in care – non clinical award
An individual or team that goes beyond their job description to help the Trust deliver its objectives and values. They will be dedicated, determined, demonstrate excellent customer care, consistently exceed expectations, think creatively and solve problems for the benefit of patient care and safety.  They will also be dedicated to quality improvements and efficiency within their service.

Improvement and innovation award
The team/ individual who have developed or undertaken service or quality improvement, made changes to the way they work, made savings or developed  education or training that will ultimately benefit patients, their department or the Trust as a whole. This may include implementing an idea or innovative approach which has resulted in improved practice, new way of working or a saving of time or money. The nomination could also result from published innovative research papers or involvement with clinical trials.

Respect and dignity award
An individual or team that values every person - whether patients, their families, carers or member of staff - as an individual.  They respect difference, understand and respect that we each have limitations, are interested in the needs of others and provide the best possible quality of care. They treat people as valued individuals and strive to put the needs of others before their own, taking into account principles of equality and inclusion.

Working together for patients award
Delivering excellent healthcare relies on great communications, teamwork and collaboration between individuals and teams.  This award will be given to individuals or teams who have demonstrated that working together leads to patient care, benefit or experience that wouldn’t be possible otherwise.

#TeamCDDFT Covid hero award
The pandemic has brought out the best of you in the worst of times. Going above and beyond what’s expected has become what you do every day.  You’ve had to work in different ways, in new roles and teams, often with little notice and been scared for your safety and that of your colleagues and family.  This award will recognise hard work but could also go to someone who has excelled in demonstrating some of the behaviours that have become so important – kindness, compassion, sensitivity.  Nominees may deliver direct patient care or be involved in providing a service essential to the delivery of that care.
