Introduction

Thank you for your help with this questionnaire.

Our Reading Record is a new initiative for veterans. We hope this will give veterans the opportunity to record thoughts on what you are reading and perhaps use this as a means to discuss books with other veterans.  The Reading Records are being distributed through organisations that support veterans and through Reading Force.

We would be very grateful if you could now complete this short questionnaire about your first opinions on the Reading Record as it will help us to develop the scheme in future. You might like to work from your copy of the Reading Record, but we have also included some pictures of it to remind you what it looks like. We estimate this will take you about five minutes.  

All the results are confidential and the survey is anonymous; we do not ask for your name. There are no compulsory questions so if you do not want to answer one, just leave it blank and move on to the next one.
