Grassroots Suicide Prevention would like to learn about how our Stay Alive app is helping people, and also how we can improve it in future upgrades (which the information you are about to give may also help to obtain funding for). The data collected in this survey is completely anonymous and confidential. You will not be asked for any identifying information (only demographic details to help us determine what sorts of population groups are using the app). When the data is analysed and published we will ensure that nothing that could possibly identify any of our participants is included so that your right to anonymity is maintained.

We recognise that completing this survey may bring up very painful memories for some people, as it will ask questions about your experiences with suicide. A link to sources of help and support will be provided at the end of the questionnaire. Please don’t be alone if you are struggling.
