We are Diabetes UK. Our vision is a world where diabetes can do no harm. We’re leading the fight against the UK's biggest and growing health crisis. And it's a fight that involves us all – sharing knowledge and taking on diabetes together.
Completing this survey is completely voluntary, but by taking part you'll be helping us to better understand and learn more about the impact of our 15 Healthcare Essentials and how we can improve and better support you and others affected by diabetes.
The results will be shared with the Cambridgeshire and Peterborough Clinical Commissioning Group (CCG) and the Local Pharmaceutical Committee (LPC). If you choose to supply your email address for entry into the Prize Draw, this will not be shared and will only be used to contact you if you are a winner. Supplying your email address is voluntary and not necessary in order to complete the survey.
This survey is hosted on the SurveyMonkey platform and is where your responses, as well as email address if you choose to supply it, will be stored. The data you supply will be deleted from the SurveyMonkey platform within 60 days of the survey closing. The survey will close on 26 October 2020.
